When the patient goes directly to the Simpleepay URL, they will be prompted for an account/guarantor number and their date of birth. However if they are directed to the pay site through an email then this is the logic for authentication:
1) If an SSN is available then we prompt for SSN/DOB.
2) If no SSN available then we prompt for Zip/DOB.
3) If SSN and ZIP are not available then we prompt for the guarantor/account number.
