The answer is: it depends.
If a patient comes from a link in an email, they will need to enter their zip code and date of birth. This is because each link sent in an email is unique to that patient so their account or guarantor number is already known.
If a patient goes directly to the Patient Portal, they will need to enter their account number and date of birth. Guarantors (if different from the patient) should enter the patient’s date of birth.
